I have a Allen & Heath Mix Wiz 16:2 dx....
It is quite common for the mixing desk manufacturers to have custom faders made up by companies like Alps, in order to provide the required taper to provide the exact control needed, correct physical size etc.
Buying from the manufacturer as Matt suggests is probably the best bet. The only caveat to that is where there is a standard, identifiable part. For example, you can add an IC to provide balanced outputs (rather than impedance balanced outputs) on the auxiliaries of some A&H desks. The ICs are quite a bit cheaper when sourced from your favourite electronics supplier.
